Mental Capacity Assessment
Assessment of cognitive function and decision-making ability for:
A Lasting Power of Attorney (LPA) is a legal document in Singapore that allows individuals aged 21 or older to appoint one or more persons (donees) to make decisions and act on their behalf regarding personal welfare and/or property and affairs matters if they lose mental capacity and are unable to make their own decisions. It ensures trusted individuals manage affairs during incapacity, avoiding court-appointed deputies.
An Advance Medical Directive (AMD) is a legal document that you can sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life.
We have doctors certified and are able to do LPA & AMD during clinic hours, please call in to check if our resident doctors are in.
We have doctors certified as severe disability assessors for severe disability assessment schemes managed by Agency for Integrated Care (AIC). These assessments can be done in-clinic or at the patient’s home. Assessment is strictly by appointment only.
Things to prepare before making an appointment
Price for first time severe disability assessment (as stipulated by AIC):
If you are already assessed to be severely disabled and have the letter from your insurer or AIC requesting for the disability assessment for review/renewal purposes, there would be no out-of-pocket costs and the assessment fee may be claimable from AIC directly.
Dementia Screening
Early detection screening for:
Includes cognitive tests and memory assessment. Further investigations including blood tests or imaging studies may be recommended following initial assessment if so required. Onward referral to specialist would also be provided if indicated.
National Adult Immunisation Schedule (NAIS) Vaccinations
Influenza (Flu) Vaccination
Pneumococcal Vaccines
Protection against pneumonia and invasive pneumococcal disease.
Recommended for all seniors aged 65 yeards and above.
PCV20 (20-valent Pneumococcal Conjugate Vaccine)
Cost: Heavily subsidised via NAIS scheme for Singapore Citizens aged 65 years and above. Low out-of-pocket cost depending on CHAS type.
Medisave: May be used to offset copayment for the vaccination costs (Singapore Citizens 65 years and above)
Shingles (Herpes Zoster) Vaccine
Cost: Subsidised via NAIS scheme for Singapore Citizens aged 60 years and above. Out-of-pocket cost depending on CHAS type.
Medisave: May be used to offset copayment for the vaccination costs (Singapore Citizens 60 years and above)

Yes, influenza vaccination is no out of pocket cost for Singapore Citizens aged 65 and above annually, under the National Adult Immunisation Schedule.
Yes, Medisave may be used for the co-pay amount for pneumococcal and shingles vaccinations for eligible Singaporean citizens. Influenza is no out of pocket money for eligible citizens.
The Chronic Disease Management Programme allows you to use Medisave for chronic disease consultations , medications and investigation. Bring your NRIC and we’ll help you access your benefits.
