What to expect during a House Call:

Before the Visit:

Mental Capacity Assessment

Assessment of cognitive function and decision-making ability for:

  • Legal documentation requirements (e.g. application for will)
  • Lasting Power of Attorney (LPA)
  • Advance Medical Directive (AMD)
Lasting Power of Attorney (LPA) & Advance Medical Directive (AMD)

A Lasting Power of Attorney (LPA) is a legal document in Singapore that allows individuals aged 21 or older to appoint one or more persons (donees) to make decisions and act on their behalf regarding personal welfare and/or property and affairs matters if they lose mental capacity  and are unable to make their own decisions. It ensures trusted individuals manage affairs during incapacity, avoiding court-appointed deputies.

An Advance Medical Directive (AMD) is a legal document that you can sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life.

We have doctors certified and are able to do LPA & AMD during clinic hours, please call in to check if our resident doctors are in.

Severe Disability Assessment (SDA)

We have doctors certified as severe disability assessors for severe disability assessment schemes managed by Agency for Integrated Care (AIC). These assessments can be done in-clinic or at the patient’s home. Assessment is strictly by appointment only.

Things to prepare before making an appointment

  • Discharge summary/memo indicating relevant medical history of patient contributing to the severe disability
  • Medication List
 

Price for first time severe disability assessment (as stipulated by AIC):

  • In-clinic assessment $100/- (inclusive of GST)
  • Housecall $250/- (inclusive of GST)
  • If the care recipient is assessed to be severely disabled, the assessment fee would be reimbursed by AIC to you.
 

If you are already assessed to be severely disabled and have the letter from your insurer or AIC requesting for the disability assessment for review/renewal purposes, there would be no out-of-pocket costs and the assessment fee may be claimable from AIC directly.

Dementia Screening

Early detection screening for:

  • Memory problems
  • Cognitive decline
  • Early signs of dementia or Alzheimer’s disease
 

Includes cognitive tests and memory assessment. Further investigations including blood tests or imaging studies may be recommended following initial assessment if so required. Onward referral to specialist would also be provided if indicated.

Vaccinations for Seniors (NAIS)

National Adult Immunisation Schedule (NAIS) Vaccinations

Influenza (Flu) Vaccination

  • No out-of-pocket costs for Singapore Citizens aged 65 and above
  • Recommended annually in general
  • Protects against seasonal flu strains
  • Reduces risk of hospitalization and influenza-related complications such as pneumonia

Pneumococcal Vaccines

Protection against pneumonia and invasive pneumococcal disease.
Recommended for all seniors aged 65 yeards and above.

PCV20 (20-valent Pneumococcal Conjugate Vaccine)

  • Covers 20 strains of pneumococcal bacteria
  • One-time vaccination 

Cost: Heavily subsidised via NAIS scheme for Singapore Citizens aged 65 years and above. Low out-of-pocket cost depending on CHAS type.

Medisave: May be used to offset copayment for the vaccination costs (Singapore Citizens 65 years and above)

Shingles (Herpes Zoster) Vaccine

  • Protects against shingles and post-herpetic neuralgia (long-standing nerve pain)
  • Recommended for adults 60 years and above, or patients 18-59 with immunocompromising conditions
  • Reduces risk of shingles outbreaks
  • Helps prevent long-term complications

Cost: Subsidised via NAIS scheme for Singapore Citizens aged 60 years and above. Out-of-pocket cost depending on CHAS type.

Medisave: May be used to offset copayment for the vaccination costs (Singapore Citizens 60 years and above)

Do you provide house calls for elderly patients?
Yes, we provide house call services island-wide for homebound elderly patients.
House call fees vary depending on location and timing required. Please contact us for specific pricing.

Yes, influenza vaccination is no out of pocket cost for Singapore Citizens aged 65 and above annually, under the National Adult Immunisation Schedule.

Yes, Medisave may be used for the co-pay amount for pneumococcal and shingles vaccinations for eligible Singaporean citizens. Influenza is no out of pocket money for eligible citizens.
